The Caledonia S. Essex Accountable Health Community is a network of community partners, established in 2013, working together via Collective Impact to improve the health and well-being of the people in Caledonia and southern Essex Counties. Collective Impact brings people together, in a structured way, to achieve social change.
The state-wide, two-day conference in Stowe provided opportunities for teams of stakeholders to learn and work together in furthering emerging or existing Collective Impact efforts. Ten members of the Leadership Team of the Collective Impact Project/Caledonia-Southern Essex Accountable Health Community worked over the course of two days in skill-building and professional enhancement workshop sessions. The Conference was sponsored by the United Way of Northwest Vermont, University of Vermont Medical Center, the Vermont Community Foundation, and the Chittenden County Regional Planning Commission. The Collective Impact Project recently received a $15,000 grant ward from the Vermont Community Foundation to sustain the Project’s efforts.
